Catamnesis

Catamnesis refers to the history of a patient following a medical or psychiatric diagnosis, or the history of a patient following his discharge from a hospitalization. For example, a person is diagnosed with bipolar disorder. Ideally, their psychiatrist/psychologist/therapist will continue to see this person and continue to keep notes about their health, well-being, progress, relapses, etc. This body of information is referred to as a "catamnesis."
